2010 FIRST Consent Forms

Hello Team

This year we will NOT be collecting paper Consent Forms, FIRST is piloting a online system:

FIRST Consent Form:
It all STARTS with the Student Application to Team!
1. Student registers in Student Team Member and Parents/Legal Guardians Information system (“Student System”) athttps://my.usfirst.org/frc/goteams/site.lasso Please click “Click Here to Get Started” under “FIRST Student Team Members”

2. As students register in Student System, Team Main and Alternate Contacts receive an email stating student has Applied to the Team

3.The “Student Team Member Summary” page, found in the “Student Team” section in TIMS, will populate with the student information from Student System including: student and parent name, their email addresses, Consent and Release Form Status/Date Complete. This information constitutes the Team Roster.

4. Main and Alternate Contacts will Accept or Decline the student, as well as select who is to be an Awards Submitter in this area in TIMS

Electronic Consent and Release Form:
1. As part of the student’s registration in the Student System, student provides an email for parent, separate and unique from his/her own

2. Parent gets email from the Student System stating child has registered and applied to team

3. Parent activates his/her own account, which will link to student’s account in Student System. This is done via link in the system generated email sent to parent

4. Once the parent account is activated, parent has logon and password to get into the Student System to complete the child’s Consent and Release Form

5. The Student System will feed information over to TIMS regarding the Consent and Release Form status (i.e., “incomplete” or “complete”) including date completed. The Main and Alternate should confirm that the status is complete for each student; if it is not, he/she should let student know to remind parent it needs to be done.
